Northwoods at Hilltop has a unique and rewarding opportunity for a Nurse Practitioner at its Niskayuna, NY inpatient medical and physical rehabilitation center for children, adults and the elderly. 

As the Nurse Practitioner, you will work closely with nurse managers and physicians to provide care to a diverse population of patients within the Traumatic Brain Injury, Ventilator Dependency and Post-Acute Rehabilitation Units.  The Nurse Practitioner is  responsible for maintaining patient care plans, attending various administrative meetings, completing medical discharge summaries and assisting with staff medical needs and physicals.
